Disability claims forms made easy

The Co-operators began using a new standard Attending Physician Statement (APS) as of February 1, 2011 according to the guidelines of The Canadian Life and Health Insurance Association (CLHIA). CLHIA has teamed up with physicians to make disability claims forms easier and all member companies have now adopted the following simplified, standardized form:

Your clients’ plan members will run into fewer obstacles when submitting a claim because insurers across the industry use this standard form to collect medical information relevant to the adjudication of disability claims. Physicians won’t have to waste time deciphering different styles of claims forms.

Physicians had limited familiarity to the multiple versions of forms in the industry.  The Canadian Medical Association has endorsed the use of a single form and has proposed that it will reduce the administrative burden so that they can spend more time with patients and less time on paperwork.

We have added The Co-operators logo on this form. Your clients can find the APS – Short Term Disability Claim form on our website. The link is called Application for Short Term Disability Benefits – Physician Statement. Your clients can now access the generic form through the CLHIA’s website on the Group Claim Forms page. The form will be evaluated for usefulness to ensure that plan members, physicians and insurers are able to process claims efficiently and effectively.

Prior to the standardization of the APS forms, the amount of time Ontario physicians spent on form completion would be equivalent to removing 1100 physicians from the province’s medical system.
